Assessments
Clear answers, put in writing.
Psychological, ADHD, WSIB CMHP and MVA assessments — thorough, evidence-based, and built to support real next steps.

ADHD program
ADHD assessment and treatment, three ways in.
Assessments include clinical interviews, psychological testing, and collateral information — report cards, questionnaires, spouse observations — so the picture is complete, not a checklist.
ADHD-I
A shorter assessment centred on ADHD symptoms and cognitive testing — for people without co-occurring conditions.
ADHD-II
Adds intellectual and academic testing — useful when learning differences or co-occurring anxiety or mood concerns are in the picture.
ADHD-III
For people already diagnosed elsewhere: symptom-based planning that shapes treatment recommendations (no new diagnosis).
Then: a treatment program that goes beyond coaching
A structured, goal-oriented program of roughly 12 sessions — procrastination, time management, organization, relationships, self-esteem, emotion regulation, school and work performance. It draws on coaching approaches but can also work with the emotions and psychological barriers underneath.
Referral-based work
Psychodiagnostic, WSIB & MVA
Psychodiagnostic assessments
A structured evaluation to clarify diagnosis and guide treatment — clinical interview, standardized psychological testing, and a clear written report with recommendations.
WSIB CMHP assessments
Community Mental Health Program assessments for injured workers, with clinicians experienced in workplace injury, chronic stress, and the return-to-work process.
MVA assessments
Psychological assessments after a motor vehicle accident, supporting treatment and insurer requirements.
Referring practitioners: physicians, WSIB case managers and legal representatives can refer directly. Call 905-455-7082 or email hello@momentaclinic.com and the intake team will send the referral form.
What to expect
How an assessment unfolds
Referral or self-contact
Physicians, WSIB and legal representatives can refer through the practitioners form; individuals can reach out directly.
Clinical interview
A thorough intake with your assessing clinician — history, symptoms, and what you want answered.
Testing & collateral
Standardized psychological testing plus collateral information where relevant — report cards, questionnaires, observations from people who know you well.
Report & next steps
A written report with findings and practical recommendations — and, where it fits, a treatment plan the clinic can carry out.
Not sure which assessment fits?
Describe what's going on and the intake team will point you to the right stream — no referral needed to ask.
